Chronic Kidney Disease (CKD), Group Education Session
Please share the registration link with patients who you feel will benefit from CKD education. Sessions are aimed at providing information to patients about their condition and how they can live well with CKD. Patients already under Nephrology do not need to be referred as they will have access to this information already. These sessions are principally aimed at patients with relatively early-stage CKD who do not require Nephrology clinic input.
